She was involved in a very serious motorcycle accident.  A relative called me regarding a Short Term Disability claim she had with AFLAC.  I printed out the claim form and took it to the hospital for her.  Since she was unable to complete the claim herself, I did so for her and found her Dr. to complete that portion required.

I submitted the claim for her and instructed her relatives regarding how to
submit future claims using the claim # provided by AFLAC and the toll free
fax # for submission. (The claim form only needs to be filled out once).

She was out of work for 6 months, but was able to pay her bills with the money provided by AFLAC.</description>
